What to share and safe contact
You may use a name or pseudonym you are comfortable with. A valid, safe contact method is needed for a reply. Say when contact is safe and whether messages may be left. An Israeli identity number is required to send the online support form and for a later eligibility check for women who hold an Israeli identity card, including those with qualifying status in a graduated procedure. Citizenship is not required; the number alone does not verify the document or current status. You may voluntarily attach photos of both sides of the card and its appendix now or arrange a safe way to provide them to a coordinator later. You may omit the description of your situation, date of birth and other optional fields. You can decline to discuss particular topics or ask us to change the contact method.
If others use your device or email, consider browser history, notifications and screen access. Closing a page does not erase browsing history. Do not send full medical records, payment details or unnecessary information about others through the general form.
Scope and limits
Completing a form does not establish a doctor–patient or lawyer–client relationship or eligibility for payment or a particular programme. Professional services, if available, require separately agreed terms and informed consent. Sharing information with a partner is discussed separately unless a legal duty or other lawful basis applies.
